Five Horizons Health Services provides comprehensive sexual health education to the community that offers a holistic look at an individual’s health through topics including HIV, STIs, physical health, behavioral health, and substance use. Our team is available to host intervention groups or speaking engagements on-site at your location.

Five Horizons Health Services provides affirming, supportive services to people with HIV (PWH). These services include social work, case management, mental health counseling, housing assistance, transportation, food vouchers, and linkage to other community resources.
As part of the HOPE Program, we also provide perinatal services to clients who are pregnant or within 12 months postpartum. Our Perinatal Social Worker offers intensive case management services designed to increase positive health outcomes and validate client experiences during the perinatal period.
