Five Horizons Health Services works to keep our communities healthy, informed, and supported. We bring health education, free testing, and prevention tools directly to the people who need them most, in neighborhoods, schools, community spaces, and local events. Everyone deserves access to the tools and information needed to prevent illness and live well.

Prevention Philosophy

At Five Horizons Health Services, we believe in extending our services outside of the clinic walls. Using our mobile unit, we travel to communities and neighborhood events in order to make care convenient and accessible.

Who We Serve

We are here for everyone, including students, young adults, people living with HIV, and anyone who does not have health insurance.

Our Services

Free HIV & STI Testing: We offer quick, private, no-cost testing along with one-on-one conversations to help you stay safe and informed.

Health Education: We provide easy-to-understand wellness education in schools, community centers, churches, barber shops, and local events.

Help Starting PrEP: We guide you through the process of starting PrEP and connect you with our clinic team to get the care you need.

Free Prevention Supplies: We provide free condoms and education on how to use them correctly to help you stay protected.

Our rapid testing options explained:

The OraQuick Rapid HIV-1 Antibody Test - Available at no cost. (OraQuick uses a mouth swab. Results are provided in 20 minutes.)

The INSTI® HIV-1/HIV-2 Rapid Antibody Test - Available at no cost. (The INSTI® HIV-1/HIV-2 Rapid Antibody Test is a rapid in vitro qualitative test for the detection of antibodies to Human Immunodeficiency Virus Type 1 and Type 2 in human whole blood, fingerstick blood, or plasma. Results are available in as little as 60 seconds.)

Five Spot by Five Horizons is a drop-in sexual health clinic offering free and confidential testing, education, community events, and wellness services focused on HIV, STDs/STIs, and sexual health. It encourages sexually active young people and adults to get tested, learn about prevention, and access support services.

Monday – Friday: 9 am – 6 pm

Saturday & Sunday: Closed

Why Five Spot?

1 in 2 sexually active young people will get an STD by age 25. Most don’t know it. Five Spot makes testing accessible, free, and stigma-free.


The clinic also provides education and resources to support better sexual health decisions. Testing takes just 5-10 minutes and is always judgement free.
