At Five Horizons Health Services, our HIV Case Management Services are designed to eliminate barriers to care for individuals living with HIV. Financial challenges, housing instability, transportation issues, and mental health concerns can all make it difficult to stay engaged in care. Our goal is to ensure clients remain consistent with medical appointments and medication, improving overall health and reducing the spread of HIV in our communities.
Medical Case Management
Our Medical Case Management team provides coordinated, client-centered support to help individuals successfully manage their health.
Services include:
- Oversight of medication adherence and medical appointment compliance
- Insurance enrollment assistance
- Drug Payment Assistance Program enrollment
Non-Medical Case Management
We offer supportive services that address social and practical needs that impact health outcomes.
Services include:
- Medical transportation
- Support groups
- Mental health and substance use counseling
- Emergency financial assistance
Housing Services
Stable housing is a critical component of overall health. Five Horizons partners with local and federal programs to provide safe, affordable housing options for eligible clients.
Housing programs include:
- Housing Opportunities for Persons With AIDS (HOPWA) – Reduced-rent housing for eligible clients
- Tenant-Based Rental Assistance Program – Reduced-rent housing for eligible clients in rural areas
- Partnership with the Tuscaloosa Housing Authority – Dedicated housing slots for eligible clients
- Kind Heart Homes – Five Horizons’ reduced-rent housing for eligible clients

Perinatal HOPE Program
Five Horizons Health Services’ Perinatal HOPE Program provides specialized support for perinatal individuals navigating HIV during pregnancy and the postpartum period. Pregnancy can involve many factors that influence health outcomes, which is why our team of medical providers, social workers, behavioral health professionals, and other supportive services takes a coordinated, whole-person approach to care.
Through collaboration between medical providers, social workers, behavioral health professionals, and supportive services staff, we help ensure individuals receive the care and resources they need throughout pregnancy and after delivery. In partnership with the UAB Family Clinic, the HOPE Program supports early intervention and preventive care to reduce the risk of perinatal HIV transmission.
The HOPE Program’s integrated Healthcare, Outreach, and Prevention services for the Empowerment of perinatal individuals navigating HIV works to reduce stigma, promote healthy outcomes, and create supportive environments where individuals and families can thrive.

Our Services
Care Coordination
Our team helps connect clients with prenatal care, HIV
medical services, pediatric care, and supportive services
to ensure continuity of care throughout pregnancy and
postpartum.
Social Work & Support Services
A dedicated perinatal social worker assists individuals
with navigating resources, addressing social needs, and
planning for a supported pregnancy and postpartum
experience.
Education & Health Planning
We provide education and guidance on pregnancy, HIV
care, postpartum health, and infant care, including
preparation for labor, delivery, and other important
transitions.
Behavioral Health Support
Licensed therapists at each Five Horizons location are
available to provide mental health support and
counseling when needed.
Collaborative Care
We work closely with obstetric providers, delivery
hospitals, and the UAB Family Clinic’s pediatric
infectious disease team to support healthy outcomes for
both parent and baby.

Behavioral Health
Five Horizons Health Services Behavioral Health Department strives to create a safe and confidential space for FHHS clients to discuss their challenges and concerns. We employ a holistic and tailored approach with each of our clients to maximize both comfort and effectiveness.
Our experienced and professionally licensed therapists provide individual therapy and psychoeducational groups to the adult FHHS population.
Who We Serve
Behavioral Health services are available to current Five Horizons Health Services clients in Alabama and Mississippi. We provide individual (one-on-one) therapy sessions. Depending on the location, services may be available in person, through telehealth, or both. Please contact your local office to confirm the options available to you.

What to Expect in Therapy
The first session will be an intake. During this session, you will answer questions about your MH history and demographics. We will also complete a Psychosocial Assessment and discuss treatment goals.
Length of Time: Sessions will be scheduled for 60-minutes
Frequency: This will be determined by you and the Behavioral Health Therapist at the end of your first session.
